What we need from you:

Experience with Med A, Med B & public Aide patient caseload

Experience in a post-acute/inpatient rehab setting is helpful

Technologically savvy — we use PCC & Optima

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.

This is an in-house Therapy Team.

via a contract company.  With the support of our discipline specific regional therapy managers, you will be a part of our first in-house team.

Salary:

$45-50/hr based on years of experience and credentials

Schedule:

MWF Days, with schedule fluctuations and weekend rotations as warranted by the center.
