Requirements
Experience with Medication Therapy Management is preferred.
Experience with prescription data entry is preferred.
Experience with Google G-Suite is preferred.
Responsibilities
Contact patients and providers to address gaps in care, with phone outreach as the primary responsibility.
This role could be dedicated to an assigned client and requires thorough documentation of all interactions and outcomes in our call tracking system.
Gather patient encounter details, including allergies, medication history, medical conditions, and other relevant information.
Contact patients to reconcile medication lists and collect information needed to support care.
Coordinate with providers and interdisciplinary teams to support medication management services.
