Discharge & Recovery Coordinator

You'll be able to manage your own caseload, complete high-quality assessments and work collaboratively with a range of professionals to coordinate safe hospital discharges and improve outcomes for the people you support.

Knowledge of housing options and relevant housing legislation.

The ability to maintain accurate records and use electronic case management systems.

We're looking for compassionate and motivated Discharge & Recovery Coordinators to join Waythrough as we mobilise the

Housing, Health & Wellbeing Hospital Discharge Support Service

Working closely with hospital teams, Adult Social Care, housing providers and other partner agencies, you'll help ensure people can leave hospital safely with the right accommodation, support and recovery plans in place.

You'll provide person-centred support both within hospital settings and in the community, managing your own caseload and working collaboratively with a range of professionals to coordinate safe and sustainable hospital discharges.

Through comprehensive assessments, support planning and ongoing recovery-focused interventions, you'll empower people to overcome barriers, improve their wellbeing and reduce the risk of homelessness and hospital readmission.

You'll also work closely with local housing authorities and accommodation providers to identify appropriate housing options, complete Duty to Refer notifications where required and ensure individuals receive timely access to the services and support they need.

Housing, Health & Wellbeing Hospital Discharge Support Service

aims to prevent individuals aged 18 and over from being discharged from hospital into street homelessness, while supporting safe, timely and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support.
