Responsibilities

The Home Health Registered Nurse is a trained professional nurse who administers skilled nursing care.

May be required to perform admission, routine, respite, or continuous care visits and assess patients.

Responsible for overall coordination of the care delivered to her/his patient caseload.

This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team and Medical Director, identifying patient, family, caregiver needs and providing those needs in accordance with the attending physician's orders and the IDG plan of care.

Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatments signed by the physician and may make the initial evaluation visit to the client.  Demonstrates competency in all skills required for the agency.

Provides initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.  Evaluates and regularly re-evaluates the nursing needs of the client; initiates, develops, implements, and makes necessary revisions to the clients plan of care.
