Requirements
Master’s degree in nursing required
Recent and relevant experience practicing as a nurse practitioner in geriatric, palliative, emergency or primary care required
Demonstrated experience with nursing staff, patient and family health teaching using adult educational principles and evidence-based practice preferred
Ability to autonomously order and interpret tests, formulate a diagnosis, develop and communicate a treatment plan for complex seniors in LTC and community settings required
Vulnerable sector check or attestation as it pertains to the Fixing Long Term Care Act required
Demonstrated knowledge of resident, patient and family-centered care
Compensation & Benefits
Hours: Currently days including the potential for weekends, evenings and statutory holidays based on program needs (subject to change in accordance with operational requirements)

Minimum: $64.02 per hour
Maximum: $79.74 per hour

Responsibilities
Accountable to the Director, Integrated Health Systems, the Nurse Practitioner will demonstrate a high level of autonomy and expert skill in formulating clinical decisions when supporting residents within central west long term care (LTC) facilities, and potentially retirement homes, congregate and other settings in the community as part of the integrated health portfolio.
The Nurse Practitioner will work to full scope of practice, utilizing all nurse practitioner core competencies (client care, quality improvement and research, leadership and education).
Support multiple LTC homes providing time sensitive acute/episodic, palliative, geriatric, and repatriation visits to LTC residents in order to avoid unnecessary Emergency Department transfers, shorten length of stay and support sustainable discharges
Build capacity of LTC staff through education and innovation as well as improving system integration through access to care, seamless transitions and partnerships
Play a leadership role by performing quality improvement, applying/performing research and best practices, participating in innovative approaches to care to improve quality and safety in LTC
Develop strong relationships with various community partners and throughout Osler and may act as a resource and consultant to individuals and groups within the professional community and other hospitals/agencies
