ACCS Registered Nurse/ LPN

Requires an associates degree in nursing

LPN or RN nursing license

Valid Massachusetts drivers license

Must possess a current RN license from the Commonwealth of Massachusetts

Schedule:

Monday-Friday, 8am-4pm

Work closely multidisciplinary team to provide education related to medical health, medications, and other related topics

Assist with identifying and accessing community resources that support client needs and wellness efforts

Guide and support clients in understanding their medical conditions

Meet with clients and staff to discuss medical diagnoses, including causes, symptoms, complications, treatment options and prevention

Provide direct medical care for clients including monitoring vitals, and wound care as needed

Review all medical documentation for assigned clients
