Training and use of the Homelessness Individuals and Families Information System (HIFIS) and CRMS Software is considered an asset;

Responsibilities

Reporting to the Supervisor, Housing Support and/or Coordinator, Housing Administration and Development or designate, this position will be responsible for:

Providing short (up to 30 days) to long term (36+ months) on-site and in-home intensive case management support to individuals and families who may be vulnerable, at risk or experiencing homelessness;

Creating and updating case plans on a regular basis;

Providing crisis intervention and clinical counselling using various therapeutic modalities;

Assessing bio-psychosocial needs and makes referrals to appropriate community resources;

Completing applications to the Justice of the Peace for Form 2 - Order for Examination under s.16 of the Mental Health Act, where appropriate;
