In this role, the selected candidate would provide exceptional care to a small panel of patients which include the following:

Deliver individualized, high-quality medical care and wellness management within a concierge model, which includes extended time for patient visits

Collaborate with a multidisciplinary team, including nurses, medical assistants and administrative staff to ensure exceptional patient care

Coordinate care with specialists and collaboratively support acute and chronic conditions

After-hours availability by phone and in-basket management with support from care team
