The Patient Care Coordinator/Intake Authorization Specialist supports clinic growth through excellence in execution of the practice management role and patient intake processes.

This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures.

The Patient Care Coordinator/Intake Authorization Specialist position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.

Convert referrals into evaluations

Schedule patient visits
