Coordinate care for long-term care residents using Ennoble Care’s EMR

Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions

Collaborate closely with Ennoble Care providers, within the long-term care setting

Serve as a key liaison between providers, facility staff, residents, and families

Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner

Accurately document patient interactions and care coordination activities

Bowie, Maryland

area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
