Responsibilities
is seeking dynamic and motivated Medical Assistants (MA) Clinical Care Coordinators to join our care team and will report to the Clinical Operations Manager.
They will support the development of patient-centered, team-based care, as well as support primary care physicians (PCPs) and practices in managing their panel of patients using a population management informatics tool.
This role focuses on high-volume patient outreach through calls, texts, and care platform messaging, while ensuring accurate documentation and seamless workflow with the care team.
In addition, this role will support key operational functions such as member enrollment, staff training, and client operations, bridging the gap between clinical expertise and operational execution.
Conduct high-volume patient outreach via phone, text, and digital platforms.
Document patient interactions and care plan updates in EHRs including Epic and Cerner.
