Responsibilities

Ennoble Care is seeking a full-time

The Care Coordinator serves as a central point of contact for coordinating patient care by managing referrals, durable medical equipment (DME), medical documentation, insurance-related requirements, and communication between patients, providers, vendors, and healthcare facilities.

This position ensures patients receive timely services while maintaining accurate documentation and supporting continuity of care.

Coordinate all aspects of patient care, including referrals, durable medical equipment (DME), medical documentation, and related administrative workflows.

Receive, review, and process physician orders to ensure all required clinical documentation, prescriptions, signatures, and insurance information are complete before submission.

Communicate with patients, caregivers, clinicians, specialists, referral offices, healthcare facilities, insurance representatives, and DME vendors to coordinate services and resolve issues.
