Planning, Organizing & Implementation

Provides direction, supervision and consultation to the clinical staff.

Clinically manages and oversees patient care by coordinating patient assignments, patient care services, and referrals

Ensures patient needs are continually assessed and that individualized plans of care are developed, implemented and updated appropriately

Serves as a resource to all agency personnel regarding clinical and operational activies

Oversees the provision of clinical care including nursing, home health aide, medical social work and therapy services consistent with stands of practice, agency policies, Medicare/Medicaid guidelines and state licensure laws.
