Responsibilities

The Clinical Pharmacy Specialist plays a vital role in delivering integrated, patient-centered care within designated outpatient clinics at the partner health system.

This position focuses on ensuring appropriate medication therapy, implementing clinical protocols and guidelines, monitoring relevant medication therapy, and enhancing the quality and delivery of evidence-based medicine.

The Clinical Pharmacy Specialist works on-site (Monday through Friday daytime shift hours) integrated with the primary care team and collaborates closely with both hybrid and onsite pharmacy service teams to maximize clinical services and coordination of care.

Conduct comprehensive assessments for disease management under collaborative practice agreements or clinic protocols.

Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models.

Assist in the development, maintenance, and appropriate use of treatment plans and supportive care plans in alignment with clinical goals and patient needs.
