Responsibilities

We’re looking for a passionate and dedicated Medical Assistant to support one of our physicians serving senior patients throughout the nation. If you’re excited about providing high-quality care coordination and building meaningful relationships with patients, we encourage you to apply.
This in-office role plays an essential part in supporting providers who are out in the field delivering in-home and community-based care. You will help coordinate care, manage follow-ups, and support chronic disease management for older adults.
Working closely with providers who are out in the field, you will help ensure care plans are implemented, referrals and labs are tracked, and patients receive consistent follow-up. This position is ideal for a Medical Assistant with experience in primary care, senior care, or home health who enjoys organization, patient communication, and supporting continuity of care.
Provide administrative and clinical support to field-based physicians and nurse practitioners
Review provider schedules and prepare patient charts prior to visits
