Experience and interest in providing health, social, and/or aging services in the community to seniors or persons with disabilities from diverse communities.

30 hours per week

Collaborates with other agencies doing similar work and with the IOA Information and Assistance Supervisor following Aging and Disability Resource Center grant guidelines.

Utilizes quality customer service skills, conducts intake and needs assessments of all clients in order to provide them with information and/or referrals to agencies, resources, or services that match their needs.

Assists clients in accessing needed services and advocates on their behalf, including but not limited to contacting agencies on behalf of clients, filling out forms, writing letters, or making phone calls to set up or confirm appointments.

Provides clients with information and/or translation services.

Refers appropriate clients to SFSC Service Coordinator.

Provides follow-up confirming that clients obtain needed services.\u00A0
