Responsibilities
to provide face-to-face assessments care to hospice patients in the Gwinnett, GA
You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.
Conduct visits with hospice patients in and near the Gwinnett GA
Complete physical exam and review of systems and assess eligibility for hospice
Speak with patients, families, and caregivers during visits and as needed
Prescribe medications supplies and appropriate medically necessary interventions
