Highly organized with excellent time management and the ability to balance patient care with productivity expectations.
Compensation & Benefits
The anticipated salary range for this role is $125,000-$150,000 annually, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.

Responsibilities
We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.
In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends.
You will oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care.
This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.
Manage a panel of patients receiving remote care, making timely clinical decisions based on evidence-based guidelines for heart failure, hypertension, and type 2 diabetes.
Deliver high-quality, evidence-based care by following established clinical protocols and workflows while applying sound clinical judgment to individual patient needs.
