Monday-Friday, 9am-5pm

Provide education and consultation to clients and staff on medical health, medications, and general medical topics

Assist clients in accessing community resources that support their health and wellness goals

Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and available treatment options

Perform direct medical care for a Person as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc.

This includes providing interventions specific to Person’s IAP (Individual action plan) for medical/health and wellness goal areas.

Review medical documentation for each assigned client and provide consultation and coordination of care as needed
