LPN - ACT Team Nurse
Requirements
\u25CF Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Louisiana
\u25CF Experience working with individuals with severe and persistent mental illness and/or substance use disorders
\u25CF Psychiatric nursing experience preferred
\u25CF Valid Driver\u2019s License in good standing and current automobile insurance
Responsibilities
\u25CF Provide nursing services as part of the ACT multidisciplinary team
\u25CF Meet with clients in homes, community settings, and occasionally the office or via phone/Zoom
\u25CF Conduct basic health assessments, including vitals and labs
\u25CF Support medication management and facilitate long-acting injectable medications
\u25CF Coordinate care with medical providers and psychiatric staff
\u25CF Support clients with severe and persistent mental illness and/or substance use disorders
