Responsibilities
Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
Assist with care transitions, referrals, and coordination with external partners, as needed

Ennoble Care is seeking a full-time
Patient Care Coordinator
to join our team in an office-based setting in the
Oklahoma City, Oklahoma
area. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.
