Responsibilities

Reporting to the Pharmacy Manager, the Pharmacist will support the pharmacy by providing diabetes education, training, comprehensive medication assessments, and medication therapy management services for patients

Build rapport and foster trusted, ongoing patient relationships over the phone and in-person

Support all responsibilities related to pharmacy operations including but not limited to medication/supplies dispensing and administration

Collaborate with prescribers and other healthcare providers to ensure coordinated care

Document consultations and follow-ups in compliance with standards of practice

Stay current with diabetes guidelines and best practices to deliver evidence-based care
