Responsibilities

They build trust through shared cultural and linguistic backgrounds, improving patient engagement and access to care.

You will educate patients on disease prevention and healthy behaviors, coordinate comprehensive care by scheduling appointments and facilitating follow-ups, and address social determinants of health by connecting patients with essential community resources.

Your work enhances patient advocacy and satisfaction and reduces healthcare costs by preventing unnecessary hospital visits.

You will also help organize community events and gather valuable health data, ensuring our care model is responsive to the community’s needs, promoting overall health equity and better outcomes for our members.
