Responsibilities
Provide counseling, assessments and group facilitation to individuals and families
Coordinate all aspects of direct clinical care from admission to discharge
Consult with the multidisciplinary treatment team to provide quality client care
Provide orientation and education to clients, families and others to aid in the therapeutic process
Collaborate with assigned clients in the development of written treatment plans; include client and family involvement in assessment, treatment, education and discharge planning
Monitor clients on an ongoing basis to continually assess needs and facilitate treatment and interventions
