Responsibilities
The Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach.
This position supports clients through all phases of homelessness, including but not limited to: on the street, interim housing, transitional housing, and permanent housing.
They assist clients with accessing and maintaining necessary services among healthcare, social services, interpersonal support systems, and navigating through a coordinated entry system (CES).
This position will provide individualized supportive services by helping each client develop a plan to address strengths, barriers, support health needs, increase income, and find and maintain housing.
Conducts intake and individualized needs assessment for all participants and collaborates to develop Individualized Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified by the participant including medical, mental health,
Provides referrals, linkages, information, and support to resources that help participants to achieve their ISP goals
