Requirements

- Bachelor’s degree in Nursing, Nutrition, Pharmacy, or related health field; Master’s degree preferred.

- Minimum of 1 year of clinical experience in diabetes education and management, preferably in an outpatient or specialty clinic setting.

- Proficiency in using electronic health records and telehealth platforms.

- Strong interpersonal and communication skills with the ability to educate and motivate diverse patient populations.

- Demonstrated ability to work collaboratively within multidisciplinary teams.

- Commitment to patient-centered care, cultural sensitivity, and ethical practice.

Responsibilities

We are seeking a highly motivated and experienced Certified Diabetes Care Education Specialist (CDCES) to join our interdisciplinary team. The ideal candidate will play a pivotal role in providing patient-centered diabetes education, support, and care coordination to individuals diagnosed with diabetes and related metabolic disorders. This role demands a passionate (CDCES) committed to promoting self-management skills, improving patient outcomes, and supporting the overall mission of Luminis Diabetes Center.

1) Conduct comprehensive diabetes self-management education (DSME) sessions for individuals and groups, tailored to patients’ clinical needs and learning styles.

2) Develop personalized care plans in collaboration with physicians, dietitians, nurses, and other healthcare professionals to optimize diabetes management.

3) Provide ongoing assessment and support to patients regarding medication adherence, lifestyle modifications, glucose monitoring, and complication prevention.
