The base salary range for this role is $113,000 to $120,000 per year based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience, physical location (state you live in) and patient care schedule. In this role, you will support One Medical senior patients across the care continuum through scheduled visits and asynchronous proactive outreach and care coordination.

You will be based in an office where seniors are seen and support patients across multiple offices.
