Requirements
Minimum 1 year of experience as a nurse in a clinical care setting

Responsibilities
The Hospice RN Case Manager is a trained professional nurse who administers skilled nursing care to patients and families who are terminally ill and meet hospice admission criteria in the home or facility setting.
May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice.
Responsible for overall coordination of the care delivered to her/his patient caseload.
This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team and Hospice Medical Director, identifying patient, family, caregiver needs and providing those needs in accordance with the attending physician's orders and the IDG plan of care.
Completing initial evaluation and re-evaluating the patient’s nursing needs during each visit.
Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
