Mental Health Case Manager - Flexible Supports

Minimum 3 years related experience in social services with adults who have a serious mental illness, dual diagnosis, developmental disability, or addictions

Experience in providing support around housing and/or homelessness

A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.

This position will provide intensive case management and advocacy services to individual’s aged sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless.

Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life.

Tasks will include direct service interventions, clinical support and community development.
