Responsibilities
The Occupational Therapist administers occupational therapy to patients on an intermittent basis in their place of residence.
This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.
Evaluate patient's functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
Participates in the development of the individualized plan of care including equipment needs
Develops treatment program and establishes goals for improved function
Communicates plan of care to referring physician and other organization professionals
