Provide supportive pre-engagement and engagement activities, including problem-solving, client education, resource referral, appointment reminders, and other obligations.

Assist in addressing any barriers that may exist.

Provide motivational counseling, resident advocacy, education, reminder calls, and outreach to encourage and facilitate residents\u2019 ability to connect with behavioral health treatment services. Arrange for interpreters (including spoken language and ASL) as needed.

Assist with setting up Paratransit or providing education on the Kitsap Transit system to assist clients with removing transportation barriers for community engagement.

Assist and support residents in finding and effectively using primary care physicians and dentists to develop health self-management skills. Work with the team to coordinate psychiatric care with medical care as needed.

Provide crisis intervention counseling and coordinate with the Mobile Crisis Outreach Team (MCOT) when appropriate.\u00A0\u00A0
