Strong background in geriatric/senior care, with a focus on care assessments and care coordination.

Savoy Life is seeking a dedicated Registered Nurse (RN) to provide hands-on clinical support for seniors in various setting including individual homes, assisted living, independent living, memory care and beyond to support direct patient care as well as extensive care coordination.

The RN will work with residents, POAs, primary care providers, community groups, on-site senior living leadership to identify care gaps and implement chronic condition care plans to improve resident outcomes.

Conduct assessments, ongoing check-ins, and other in-person care for residents within senior living communities and other senior setting to identify and support care needs.

Assist Savoy’s Advanced Practice Providers in developing and implementing chronic condition care plans in collaboration with the clinical team.

Coordinate care for residents through collaboration with primary care providers, specialists, POAs/families, and other care services to ensure seamless patient care delivery.

Partner with an internal team including a local Resident Care Coordinator, Partner Success leader, and virtual Advanced Practice Provider.
