Provide physical therapy services to patients according to a plan of care.

This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.

Instruct patients, families and caregivers in the use and care of therapeutic appliances.

Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.

Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.

Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
