Requirements
Care navigation experience
— 2–5 years minimum in care navigation, case management, care coordination, or a closely related patient-facing role
— comfortable navigating social determinants and connecting patients to resources across complex systems
— comfortable with care management platforms, EHRs, and digital tools; quick to learn new systems
Experience in a startup, pilot program, or ground-up initiative
Familiarity with value-based care or population health models

Responsibilities
This is a rare opportunity to be the first clinical hire at a growing care navigation program.
In this dual role, you'll carry an active caseload of patients while working alongside leadership to design the infrastructure, workflows, and standards that will define how we deliver care at scale.
As the program grows, so will you — with the potential to grow into a leadership role overseeing a multidisciplinary team of advocates, CHWs, and clinical staff.
