Responsibilities
\u2022 Complete required documentation for newly admitted clients and establish collaborative relationships with collateral contacts, including nursing staff, prescribers, and families.
\u2022 Develop and maintain individualized treatment plans for voluntary and involuntary residents, incorporating relevant medical and nursing information.
\u2022 Participate as a member of the multidisciplinary treatment team to develop and implement treatment plans that address acute needs appropriate for inpatient care.
\u2022 Develop discharge plans for voluntary and involuntary residents, including arranging and documenting seven-day follow-up appointments and coordinating with referral sources.
\u2022 Provide individual, group, and family therapy services and maintain accurate, timely documentation in accordance with regulatory, agency, and funding requirements.
\u2022 Complete screenings, assessments, intakes, and discharges and coordinate aftercare services.
