Compassionate, adaptable, and reliable team player

Comfortable working independently and in diverse environments

Medical Assistant diploma preferred

Experience or education in patient care or case management

Valid driver’s license and ability to travel locally

Visit patients in their residential homes to support care plans

Create individualized care plans to meet the needs of each patient

Assist with dementia program activities and telehealth visits

Provide care coordination and resource support

Build relationships with patients, families, and community partners

Support documentation, scheduling, and community outreach events
