Hospice RN Case Manager

Minimum of one (1) year of nursing experience.

Previous hospice, palliative care, home health, oncology, or case management experience.

Strong clinical assessment and care planning skills.

Experience educating patients and caregivers.

A compassionate, patient-first mindset and a commitment to delivering dignified, person-centered end-of-life care.

🩺 Deliver Compassionate Clinical Care

Complete comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs.

Develop and manage individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.

Monitor patient progress and adjust care plans as needs evolve to maximize comfort and quality of life.

Serve as the primary clinical resource for patients and families throughout their hospice journey.

Assess and manage pain and other end-of-life symptoms using evidence-based clinical practices.
