Responsibilities
The Advanced Practice Provider (APP) collaborates with a multidisciplinary healthcare team and is service aligned, managing a defined patient population in the outpatient setting.
This role emphasizes general, episodic, and preventive care while delivering patient-centered services.
A strong foundation in evidence-based practice and education is essential, as the APP ensures adherence to clinical pathways, protocols, and fosters an environment of safe, high-quality care.
The provider will demonstrate advanced medical knowledge, clinical and technical proficiency, sound clinical judgment, effective professional communication, and timely, compliant documentation.
Additionally, the provider actively engages in professional development, competency validation, and contributes to the continuous improvement of patient care standards.
In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses.
