Requirements
Minimum one (1) year experience as a nurse in a clinical care setting
Nursing skills as defined by accepted nursing standards
Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles

Responsibilities
Optimal Care Hospice Registered Nurses (RNs) are skilled professionals who provide comprehensive nursing care, case management, and compassionate support to terminally ill patients and their families in home or facility settings.
As a Hospice RN, you will assess, plan, coordinate, implement, and evaluate individualized patient care plans.
Working closely with the Hospice Medical Director, you will lead an Interdisciplinary Team—which may include Hospice Aides, Medical Social Workers, Chaplains, LPNs, Music or Massage Therapists, and Volunteers—to ensure patients receive holistic, high-quality care.
Conduct initial patient evaluations and ongoing reassessments to meet nursing care needs.
Manage your caseload, including assessment, planning, coordination, implementation, and evaluation of patient care plans.
