Responsibilities
Manage the full intake and referral process, from initial referral through admission
Receive, review, and convert patient and physician referrals in accordance with agency policies
Accurately enter, track, and maintain referral and patient data in company systems
Serve as a key point of communication between patients, referral sources, clinical teams, sales, and leadership
About the Company
Coordinate referrals with supervisors and schedulers to support timely patient evaluations
Maintain organized systems to ensure continuity and quality of patient care
Provide excellent internal and external customer service, including greeting visitors professionally
Prepare referral tracking and trending reports as needed
Credential new physicians and facilities following established procedures
Identify workflow gaps and recommend improvements to enhance efficiency
