Responsibilities

Number of vacancies:

Provide advanced clinical practice consultation for the care of patients with complex needs

Facilitate the implementation of best practices in relation to the complex care issues of the patient population;

Engage with patient care team to foster, facilitate, enhance, and support best practice for the Acquired Brain Injury and Stroke patient populations

Work with the patient care team to promote goal-setting and patient-centred care

Provide coaching and mentorship to enable the evaluation and application of best information in the planning and delivery of care
