Requirements
Must have experience in direct clinical care, including intake functions/triaging as well as in community relations and service development

Responsibilities
Number of vacancies:
As an integral member of the Cardiac Rehab team, the responsibilities of the CLINICAL INTAKE COORDINATOR, include:
Utilizing clinical knowledge and experience to serve as an entry point for the timely access to appropriate clinical assessment and treatment services in cardiac rehab.
Reviewing patient cases to decide if they are clinically ready for rehab.
Working along with the Service Coordinator to Triage the receipt of approximately 75 referrals per week.
Obtaining and maintaining current knowledge of internal/external services to communicate timely information.
