Requirements

Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.

Experience providing case management and rehabilitation services.

At least 2 years experience working with system-involved youth.

Compensation & Benefits

$34.37 per hour (Rate inclusive of 10% Spanish language differential)

The Team-Based model of care program named

EMPOWR

(EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

Responsibilities

draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth.

Understand most spoken Spanish in professional and conversational contexts, using context to interpret unfamiliar slang, idioms, or complex grammar

Access a growth-oriented learning community:

Team

Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living.

Provide intensive skill building with youth, with a focus on independent living.

Provide assessments, crisis intervention, stabilization, and safety plans.

Identify and connect youth & young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities.

Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.

Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.
