In the provision of patient and non-patient care activities, the primary responsibilities of the SOCIAL WORKER in the inpatient Geriatric, MSK, and Multi-System Rehab include:

The innovative application of social work processes in psychosocial assessment, education, supportive and adjustment counselling with patients and families;

Conducting assessments independently and as part of an interprofessional team; coordination of care plans and future care planning, including appropriate referral to community agencies and support services;

Collaboration with the interprofessional team and community-based service providers; providing leadership related to team communication with patients and families;

Participation in professional, program and hospital-wide program development and meetings;
