Oncology Nurse Navigator - Full Time - Wolf River

At least 1 year of nursing experience (required)

Outpatient nursing and scheduling experience (preferred)

Strong knowledge of oncology practices and patient navigation

Compassionate, patient-centered approach with strong attention to detail

The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum.

This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care.

ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.

Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs

Advocate for timely, high-quality care by guiding patients through the healthcare system

Coordinate appointments, referrals, and services to ensure continuous care
