Responsibilities
Working as part of a multidisciplinary team, you'll manage your own caseload while coordinating support across a range of partner agencies to ensure people receive the right help at the right time.
You'll work closely with individuals, families and wider support networks to develop personalised recovery plans, deliver evidence-based interventions and support people to access services that improve their health, wellbeing and independence.
Manage a complex caseload, delivering comprehensive assessments, personalised recovery plans and evidence-based interventions.
Maintain accurate case records and outcome data in line with organisational policies and contractual requirements.
