Requirements

Bachelor\u2019s Degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology, or a related field plus two (2) years of direct care experience OR
HS Diploma/GED and six (6) years of full-time direct care experience.
Minimum of one (1) year of experience providing employment/rehabilitation-related services and support to individuals with disabilities required.
Must have reliable transportation, valid AZ Driver\u2019s License with state minimum insurance coverage and ability to meet and maintain Copa Health\u2019s acceptable driving requirements.
TB test clearance required at the time of hire.

Responsibilities
As a member of the Integrated Care Team, participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members\u2019 progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.
