Responsibilities
Optimal Care Providers (Nurse Practitioners and Physician Assistants) provide Primary Care and/or Consultative Professional Care in a variety of care settings.
The Provider will function in collaboration with Physicians according to a Collaboration Arrangement.
The Provider's practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
Determining follow-up assessments according to patient needs and best practice guidelines
Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care
