Nurse Practitioner
position at Center for Elders’ Independence is $138,861 - $222,178
per year
. Placement within the range is determined based on job-related factors such as relevant experience, skills, abilities, internal equity, and market data, consistent with CEI’s annual compensation review and established compensation practices.
Responsibilities
The Nurse Practitioner, reporting to the CMO and working under the supervision of a PCP, provides medical services to a diverse, frail and elderly population at one or more of CEI's primary care clinics and is a critical part of an Interdisciplinary Team (IDT) operating under the "PACE" model.
acute health care problems including illnesses, infections and injuries, provides urgent care for acute medical conditions and may order and/or perform diagnostic tests.
Provides routine healthcare to participants, including conducting semi-annual and periodic assessments of participants and furnishing/ordering medications and devices.
Analyzes and interprets participant histories, symptoms, physical findings and diagnostic information to develop appropriate diagnosis.
Develops treatment plans based on scientific rationale, standards of care and professional practice guidelines and participates in IDT development of comprehensive care plans.
Counsel’s participants on drug regiments, possible side effects, interactions with other substances, interventions to modify behavior associated with health risks and may offer counsel on how best to follow the care plan.
