Responsibilities
to join our Care Management team with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge.
This position works closely with all members of the interdisciplinary health care team, ensuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient.
Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care.
Lead medical team and nursing huddles and work creatively on removing barriers to discharge due to medical complications and/or system or team obstacles.
Assist with ordering complex medical equipment and in ensuring safe transitions of care.
Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning and providing ongoing consultation and education to staff on how to achieve improved patient flow through the UWMC system.
